Personal Request for FBI Criminal History

When you apply for a visa to reside, work, or run your own business in Belgium, the Belgian authorities may ask you for a police certificate, commonly referred to as a “certificate of good conduct” in Belgium.  To obtain such a certificate, American citizens and/or foreigners who have resided in the United States need to contact the Federal Bureau of Investigation and submit a request for an FBI criminal history as follows: more

Prepare your written request with:

  • a form FD-258 with your fingerprints on it;
  • your return mailing address;
  • a photocopy of a picture ID (preferably a U.S. driver's license);
  • a letter stating the purpose of the record check;
  • the required fee of $18.00 which must be submitted in the form of a certified check or money order, payable to the Treasury of the U.S., or with credit card payment
