Lost or Stolen Passports
Under 16 years
Please bring:
- police report: If your passport was lost, stolen or destroyed, you should report it to the local police immediately and obtain a police report, because when replacing a lost, stolen or destroyed passport, we need to send a police report to the Department of State in Washington, D.C.
- proof of citizenship; e.g. U.S. birth certificate, Consular report of Birth, naturalization certificate,…
- proof of identity: e.g. foreign passport, foreign ID-card, U.S. driver's licence, copy of the most recent U.S. passport
- one U.S.-size photo (5 cm by 5 cm or 2" by 2"). Attention: for technical reasons no self-printed passport pictures can be accepted by the consulate! The passport photograph you want to submit to the Passport & Citizenship Unit for you to obtain your passport, should comply with following guidelines:

One unsigned photograph taken within the last six months.

2x2 inch.(5x5 cm) square, with the image measured from chin to forehead not less than 1" or more than 1-3/8".

Photo should not be cut.
- Color photos only will be taken in consideration; not tinted, with a light background.

fee: $105
- a completed passport application form along with a statement regarding lost or stolen passport (PDF - 67.4KB). (This form should be downloaded along with the DS-11 Application for Passport and brought with you when you go to apply for a replacement passport). When completing the Form DS-11, include the social security number (SSN). The application cannot be processed if the SSN is missing.
- All minors age 16 and over must appear in person to sign their application. All minors under age 16 must also appear in person and their applications require the signature of both parents.

To have your new passport returned to you by registered mail, please bring a self-addressed envelope per person, with Belgian registered mail postage of €4.70 (aangetekend/enregistre) + 1 stamp . Please do not use machine-printed meter tape, but regular stamps.
