MAKE SURE TO MAIL YOUR PASSPORT APPLICATION DS-82 TO :

You may apply by mail using Form DS-82 if:
- You can submit your most recent passport;
- You hold a passport valid for 10 years issued within the last 15 years; and
- You are not requesting a change in the passport data, other than a name change due to marriage or divorce for which you can submit official documentation (marriage certificate or court order) to support the name change;
- You were at least 16 years old when your most recent passport was issued;
- You have one of the following credit cards : VISA, MASTERCARD, MASTERCARD, AMERICAN EXPRESS, DISCOVER.

In Person (BY APPOINTMENT ONLY)
All other persons must apply in person and complete Form DS-11, except if you are the holder of a passport valid for 10 years issued within the last 15 years, in which case the same rules apply as for an application by mail (please see the procedure outlined above). When completing the Form DS-11, include the social security number (SSN). The application cannot be processed if the SSN is missing.
Under 16 years of age (BY APPOINTMENT ONLY)
Please bring:

a proof of U.S. citizenship; e.g. U.S. birth certificate, consular report of birth, naturalization certificate, ...

one U.S.-size photo (5 cm by 5 cm or 2" by 2" - (list of specialized photographers). Attention: for technical reasons no self-printed passport pictures can be accepted by the consulate! The passport photograph you want to submit to the Passport & Citizenship Unit for you to obtain your passport, should comply with following guidelines:

One unsigned photograph taken within the last six months.

2x2 inch.(5x5 cm) square, with the image measured from chin to forehead not less than 1" or more than 1-3/8".

Photo should not be cut.

Color photos only will be taken in consideration; not tinted, with a light background.

- a completed passport application form
- proof of identity (if available); e.g. foreign passport, foreign ID-card,…
- All minors age 16 and over must appear in person to sign their application. All minors under age 16 must also appear in person and their applications require the signature of both parents.

To have your new passport returned to you by registered mail, please bring a self-addressed small POSTPAC envelope or equivalent (220mm x 152mm) per person, with Belgian registered mail postage of €4.70 + 1 stamp . Please do not use machine-printed meter tape, but regular stamps.
Over 16 years of age (BY APPOINTMENT ONLY)
Please bring:
- proof of citizenship; e.g. U.S. birth certificate, Consular report of Birth, naturalization certificate,…
